You have to go to https://ads.google.com/local-services-ads/ and sign up for it, the information they will need is listed here. You need proof of ID, insurance, and an existing Google Map address with at least 1 review already.

• General Information – Number of people in business, years in business, and business name to show along with the Google Map address you have for your business
• Proof of Insurance – you need proof of insurance; the amount minimums vary by each business industry
• Reviews – need at least 1 Google map review, and the system checks for the review and the legitimacy of the review
• Evident.com linked – takes 1-2 weeks for approval –  they need the owner’s social security number
• Photo ID – The system sends a request for a Photo ID to the customer’s (your) cell number; they (you) have to take a pic of your ID on the spot
